SEMA / QUESTION KIT

Questions to help you find and check care

A guide for your next care conversation

What can I take to the visit?

Keep the questions that matter to you. Print this page or copy them onto paper, leaving room for the health professional’s answers.

Keep in mind

Choose and carry your care questions

Circle the question you want answered first. A worry about money or a concern about the medicine belongs in the conversation.

You can say, “Please explain that word,” or, “Let me say back what I heard.”

Beyond the name you found

Questions about the person and the product

  • Who will make the care decision, and which licence covers my state?

  • Please name the medicine in full and tell me its form.

  • Why does that product fit the health problem we are discussing?

  • If it is made to order, what change is needed in my case?

  • Which pharmacy would make it, and which would supply it to me?

Keep in mind

Get an explanation you can repeat

Please explain the plan so I can tell it back in my own words.

Write down what is decided and what is still open. Ask which person handles the unanswered parts.

Beyond the name you found

Questions about your health, payment, and later care

  • What in my health history changes this choice?

  • What needs checking before a decision, and who arranges it?

  • What is the whole bill now and after any offer ends?

  • What does my plan need before it will pay?

  • How do I ask about unwanted effects, and when is urgent care needed?

  • How will we check progress, and what if the plan changes?

The semaglutide label connects health-history checks with thyroid cancer, pancreas and gallbladder problems, pregnancy plans, and surgery.

Bring that health history to the conversation.
